Empowering Patients

  • Engage the patient in self managing of chronic conditions through simple, easy to read goals.
  • Empower the patient to know the “red flags” or warning signs and symptoms for their episode and how to respond.
  • Promote not only preventive health, but disease control and health promotion.
  • Encompass the entire episode of care into one booklet allowing for multiple steps in one visit.
  • Inform the patient to prepare the patient for the next visit and to allow for a reduction in visits.
